RN - Home Based Primary Care, Chronic Care (FT) 8a-5p Tri-Cities area

SCOPE OF POSITION
The Registered Nurse (RN) is responsible and accountable for the provisions of nursing care for assigned patients within the established standards of evidenced-based practice and organization policy and procedures. The RN will provide in-home assessments for patients with chronic conditions, who may also be acutely ill or recently discharged from the hospital or emergency department. The nurse is expected to practice autonomously within the scope of professional standards and rules and regulations established by the TN and/or VA State Board of Nursing.
Key Responsibilities:

  • Conduct thorough and comprehensive assessments of patients in their homes, focusing on both chronic and acute care needs.
  • Develop personalized care plans based on patient needs and assessment findings with guidance from the primary care provider.
  • Demonstrates the ability to use critical thinking skills related to the clinical RN role where assigned.
  • Determine and recommend appropriate follow-up care, including scheduling visits with primary care providers (PCPs) and specialists as necessary.
  • Facilitate telemedicine visits between patients and their PCPs during home visits, ensuring smooth and efficient communication and care coordination.
  • Utilize electronic medical records (EMR) systems to document patient assessments, care plans, and all relevant interactions accurately and timely.
  • Educate patients and their families about disease management, medication administration, and other aspects of home care.
  • Monitor patient progress, identify changes in condition, and adjust care plans as needed.
  • Communicate effectively with the healthcare team, including physicians, nurse practitioners, and other allied health professionals, to ensure comprehensive and coordinated care.
  • Provide compassionate and empathetic care, building trust and rapport with patients and their families.
  • Travel locally to patient homes, adhering to all safety and infection control protocols.

EDUCATION AND EXPERIENCE

  • Current and unrestricted Registered Nurse (RN) license. (Licensure in both Tennessee and Virginia is needed.)
  • Minimum of 2 years of clinical nursing experience, preferably in home health care.
  • Strong clinical assessment and critical thinking skills.
  • Proficiency with technology, including telehealth platforms and electronic medical records (EMR) systems.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Valid driver’s license and reliable transportation.
  • CPR certification.
  • Ability to adapt to varying home environments and patient needs.
