+ **On Call:** No **The Case Manager is responsible for:**
+ Providing linkage, monitoring, planning and advocacy for the most appropriate mental health and community medical and social services.
+ Coordinating the service delivery system for patients.
+ Providing residents, faculty and community mental health practitioners with up-to-date available referral resources to provide continuity of care.
+ Performing other duties as assigned. **Required Education:**
+ Bachelor's in Related Field, Human Services Preferred **Experience:**
+ 1 year in Social Work requiredOr - 1 year - Case Management required **Required Certification/License:**
+ BLS (Basic Life Support)
