Provide direct patient care and comprehensive coordination of care for patients and their families/caregivers, to eliminate barriers to timely care, facilitate flow through system, facilitate interactions with clinical care team, increase patient and provider satisfaction, increase patient quality, maintain point of contact with providers and serve as the first point of contact for patients and families.
Track and allocate oncology patient referrals to ensure timely service according to the needs of the patient considering patient/referring provider preferences.
Track and record quality metrics assigned to the oncology service line
Coordination of Rectal Cancer multidisciplinary clinic and accreditation.
Nurse navigation duties for rectal cancer patients
**Responsibilities**
**Bergan Mercy Oncology Department is hiring a Full-Time Nurse Navigator to support our Oncology Department.**
**Hours Monday - Friday 8:00 am to 5:00 pm.**
1. Educate patients about their diseases, treatment, and side effects of treatment and how to manage them.
2. Provide clinical resources – where to seek a second opinion, treatment or testing that may not be available locally, and how to access reliable sources of information.
3. Assess patients for psychosocial distress and referral to a medical social worker or counselor if more intensive support is needed.
4. Answer questions as appropriate and serve as a liaison to the physician for additional questions and concerns.
5. Address tangible barriers to care such as finances, housing, transportation, childcare, prescription costs, and much more. They refer patients to the appropriate contact, such as a social worker.
6. Ensure any health information provided is accessible: this may include securing documents printed in the patient’s own language or arranging for a certified medical interpretation, either in-person or by phone/video conference.
7. Function as a member of an interdisciplinary team to assess, secure and coordinate appropriate services through the continuum of care.
8. Empower patients, families, and caregivers to assume as much responsibility for their care as possible, with the constraints of age, illness and other factors.
9. In consultations with other members of the healthcare team, promote awareness of clinical trials as appropriate.
10. Support the timely scheduling of appointments, diagnostic testing, and procedures.
11. Educate/identify patients for genetic counseling or fertility preservation, and facilitate appropriate referrals.
**Qualifications**
3 years clinical nursing experience
Active RN licensure in the state of Nebraska, Iowa, or nurse licensure compact.
Basic Life Support is required upon hire. Acceptable credentialing bodies and certifications include the following: American Heart Association: Basic Life Support.
**Pay Range**
$33.51 - $48.58 /hour
