Clinical Pharmacist, Heart Failure & Specialty Pharmacist A split duty role with duties evenly weighed between working directly in the FirstHealth Heart Failure Clinic while also remotely performing support for the FirstHealth Specialty Pharmacy. At times, may be asked to cover main duties of specialty pharmacy within the Outpatient Pharmacy. A non-exhaustive list of disease states needed to cover to be successful in this role include Heart Failure, HIV, Hepatitis C, Oncology (solid tumor, hematologic cancers, supportive therapies), and Neurology (multiple sclerosis, migraines, pain). Responsibilities and Scope · Educates patients regarding medication details and disease states. Provides initial clinical medication counseling and regular patient reassessments. · Provides initial clinical medication counseling and regular patient reassessments via in person, telephonic, or video encounters. · Performs prior authorizations, financial assistance, or other necessary enrollments to increase medication access for patients. · Adjudicates and fills prescriptions as needed. · Maintains services in line with operational policies and procedures, workflows, and practice expectations. · Provides limited/indirect supervision for Specialty Pharmacy Technicians and timely response to clinical escalations. Requirements: · Graduate of an accredited school of pharmacy. · Post-graduate pharmacy residency or equivalent pharmacy experience required. · Currently licensed as a Pharmacist by the North Carolina Board of Pharmacy. · Ability to work Full Time Preferred but not required: · Experience with Epic electronic medical record, Rx30 pharmacy system, and Asembia patient therapy manager. · Credentialed by the Board of Pharmacy Specialties (BPS), National Association of Specialty Pharmacists (CSP), or other appropriate nationally recognized pharmacist credentialling board. Candidate must exhibit: · Excellent customer service and communication skills (patient counseling; interaction with other care team members; remotely describing tasks needed to other staff). · Self-driven ability to maintain quality therapeutic knowledge. · Support of FirstHealth quality initiatives surrounding the care of patients. · Proficient computer skills including MS Office Suite (Word, PowerPoint, Excel, and Outlook).
