Pediatric Transplant Pharmacist

**Job Description:**

This position provides appropriate and effective clinical pharmacy care, leadership, mentoring, and training to clinical staff, pharmacy residents, and pharmacy students. Incumbents are expected to actively contribute to the organizational knowledge and goals of Intermountain Healthcare and to the scientific body of knowledge.

Schedule

Monday – Friday with rotating on-call weekend service

Job Essentials

Performs all patient care duties and expectations of a Pharmacist-Clinical.

Provides leadership to system clinical pharmacy networks and Intermountain Clinical Programs in area of expertise, as well as local leadership and expertise with the goal of clinical excellence, quality improvement, and system integration.

Actively involved in the development and implementation of protocols, clinical pathways, guidelines, and medication use evaluations.

Actively collaborates or conducts research with intent of publishing findings to contribute to the scientific body of knowledge.

Act as a preceptor for a minimum of six learners per year (based upon availability of learners).

Actively elevates the practice of Pharmacy by speaking at state or national pharmacy meetings, filling committee assignments in state or national professional organization, serving on boards of state or national organizations, or participating on hospital or system committees.

Actively involved in professional development of general pharmacy staff as well as education of other disciplines (nursing, physicians, medical residents, etc).

Minimum Qualifications

+ Current license as a Pharmacist in the State of practice.

+ Controlled substance license.

+ Maintains board certification by the Board of Pharmaceutical Specialties in area of practice (if no such specialized certification exists then must have board certification in pharmacotherapy).

+ Basic Life Support Certification (BLS) for healthcare providers.

+ Must meet one of the following three requirements: Five years of experience working as a pharmacist in a patient focused clinical practice setting

+ or

+ a doctor of pharmacy degree plus one year of post graduate training in an accredited pharmacy residency program plus one year of clinical practice

+ or

+ a doctor of pharmacy degree plus two years of post graduate training in an accredited pharmacy residency program.

+ and

+ Current knowledge of state and federal regulations and statutes regarding the practice of pharmacy and the distribution of approved drugs, controlled substances, and investigational agents.

+ and

+ Three years of experience mentoring, training, or acting in a preceptor assignment evidenced by meeting the ASHP preceptor requirements.

+ -and

+ Obtain and maintain an appointment as adjunct faculty within at least one college of pharmacy.

+ -and

+ Present yearly in one of the following: Local or regional P&T, System P&T, System-wide or local professional development program.

+ Present yearly in one of the following: Local or regional P&T, System P&T, System-wide or local professional development program.

Preferred Qualifications

Leadership experience.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.

$59.86 - $87.98
