BayCare Health Systems is currently seeking an experienced **Case Manager** to join our outstanding and compassionate team in Winter Haven **!** This is a field-based position that works throughout all of Polk County.

+ **Location** : Winter Haven

+ **Status:** Full-Time, Hourly

+ **Shift Hours:** 8:00 am-4:30 pm

+ **Weekend Work** : No

+ **On Call:** No

+ **Remote:** Hybrid

**The Case Manager is responsible for:**

+ Providing linkage, monitoring, planning and advocacy for the most appropriate mental health and community medical and social services.

+ Coordinating the service delivery system for patients.

+ Providing residents, faculty and community mental health practitioners with up-to-date available referral resources to provide continuity of care.

+ Medical Care Coordination

+ Performing other duties as assigned.

+ Flexibility and adaptability are also essential for this role, given the ever-changing nature of our work and the need to respond to changing client needs and program requirements.

**Required Education:**

+ Bachelor's in Related Field

+ Preferred Bachelor's in Psychology, Social Work, Human Services or a related Mental Health Field

**Experience:**

+ 1 year in Mental Health required

**Required Certification/License:**

+ BLS (Basic Life Support)

+ Driver's License
